Why our program could be right for you

The Montefiore Einstein Multidisciplinary Pain Fellowship is the largest ACGME accredited PMR Pain program in the country. Our mission is to improve function and decrease pain for our patients through responsible pharmacological mindedness, sound clinical judgment, exceptional science and the integration of the newest and most advanced interventional techniques.

Our fellows get early operative experience and quickly develop autonomy in the evaluation and management of patients. We are one of the highest-volume spinal cord stimulation (SCS) programs in the nation. Fellows have the opportunity to perform a large number of image-guided procedures, and are trained in percutaneous surgery techniques such as minimally invasive lumbar decompression (MILD), percutaneous disc decompression and kyphoplasty.

The Montefiore Multidisciplinary Pain Medicine Fellowship is a 12-month, ACGME-accredited program and one of the largest Physical Medicine and Rehabilitation (PM&R)-based Pain Medicine fellowships in the country. Our program offers a robust, evidence-based curriculum designed to prepare the next generation of leaders in Pain Medicine through a unique blend of clinical excellence, academic rigor, and interdisciplinary collaboration. The fellowship is primarily based at the Montefiore Hutchinson Campus, providing fellows with access to state-of-the-art facilities and a diverse patient population within one of the busiest healthcare systems in New York City. Fellows benefit from comprehensive didactic training—including basic science, translational research, multidisciplinary pain conferences, and weekly board preparation—while gaining extensive hands-on experience in the latest interventional procedures. The program is deeply committed to academic scholarship, with active involvement in clinical trials and research. In addition, fellows receive education in the business and administrative aspects of pain medicine, promoting a well-rounded foundation for future practice. With a mission rooted in improving function and reducing pain through responsible and compassionate care, Montefiore continues to attract top candidates nationally and remains a leader in Pain Medicine training.

Graduate Medical Education at Montefiore Einstein

Representing one of the top 10 largest medical and surgical training programs in the country, Montefiore Health System and our Albert Einstein College of Medicine provide postgraduate clinical training to more than 1,400 residents and fellows per year. Clinicians across Montefiore Einstein’s numerous educational programs actively mentor and cultivate a new generation of healthcare leaders.

To make the process of working and training at Montefiore Einstein more seamless, we offer a variety of services for our house staff, physician assistants and interns, including assistance with credentialing and orientation, institutional resources, visas, contracts, HR documents, payroll, notary services and specialty training and certifications.

An ambulatory surgery center and specialty care location

Our world-class Hutchinson Campus raises the bar for efficiency and innovation in healthcare. Designed with our patients in mind, the 11-floor, 280,000-square-foot facility is a patient-focused, highly streamlined ambulatory care center that maximizes efficiency. Our fellows spend approximately 10 months out of the year in this facility.

Located on the eighth floor, our clinic performs a variety of office-based procedures, including ultrasound guided musculoskeletal (MSK) injection, nerve injections, peripheral nerve stimulation, and more. Our ambulatory surgery center is adjacent to the clinic, where we perform our fluoroscopic procedures and percutaneous surgeries.

A partner teaching hospital designated as a Level 1 trauma center

A partner of the Montefiore Health System, Jacobi Medical Center (JMC) is one of the best and most recognized trauma centers in the world. Our fellows split time working with anesthesiologists in the operating room, in post-anesthesia care units (PACUs) and on medical surgical floors, learning regional anesthesia and acute pain management. While training at JMC, fellows also work with rehabilitation doctors in our clinics and fluoroscopic suites. Fellows will spend 2 months in this site.

Our busiest and largest location, Moses Campus is the main campus of Montefiore Einstein and features 721 adult beds. Fellows rotate with some of the best addiction psychiatry and palliative care physicians in the country, rounding out their education in pain medicine.

A challenging and innovative curriculum

The Montefiore Einstein Physical Medicine and Rehabilitation (PM&R) Pain Medicine Fellowship is nationally recognized for training outstanding pain practitioners through its excellent clinical education, knowledgeable staff and accommodating working environment.

Clinical Rotations: Our 12-month (July-June) program consists of 6 two-Month Core Rotation Blocks. In outpatient blocks A-E, fellows are paired with the attending physicians listed below. During a typical week in clinic, fellows have an average of 2 days dedicated to procedures and 3 days for regular clinic visits. The day typically starts at 8:30am and finishes around 4:30pm, with some variability. Additionally, fellows will have valuable exposure in specialized areas such as the palliative care clinic, as well as dedicated rotations in neurology/headache clinic for one week, and addiction psychology for another week.

Interventional Pain

Our program is one of the most comprehensive academic interventional pain programs in the world. Fellows learn to treat patients using cutting-edge technology and the principles of evidence-based medicine. Supported by our highly-skilled staff, fellows train in a robust and fast-paced clinical environment.

Psychiatry

Our fellows rotate through our nationally renowned Addiction Psychiatry Program, where they learn to manage chronic pain in an addiction-focused setting, commensurate with the mandates of national guidelines and policies.

Palliative Care

During their training, fellows work alongside caring, knowledgeable and diligent palliative care physicians to learn the importance of chronic pain care used in conjunction with opioid management and end-of-life care.

Acute Pain

Our fellows learn to manage acute pain in the inpatient and acute settings while rotating through our partner hospital, Jacobi Medical Center, an institution internationally recognized for its expertise in emergency and trauma medicine.

Neurology/Headache Management

Fellows have the opportunity to train within the Montefiore Einstein Headache Center, the world’s first academic headache program, known for its outstanding academic environment and clinical chronic headache care. Along with our Chronic Pain Center, the Montefiore Einstein Headache Center is where our fellows train to understand the medical management of headaches. 

Didactics

Fellows have protected didactics time, including fellow-led, attending precepted lectures on Monday mornings at 7:30am. Additionally, board review sessions take place on the 2nd and 3rd Thursdays of each month. These sessions are thoughtfully designed to support fellows' learning and preparation.

Electronic Medical Record

EPIC is the EMR for all sites.

Call Schedule

Throughout the year, fellows are assigned a one-week call approximately every 5th week, with their primary focus being the timely handling of urgent outpatient matters from 5pm to 7am. On average, fellows typically receive a minimal amount of 0-4 calls during their dedicated one-week call period. Fellows do not have any call responsibilities during the Jacobi Medical Center rotation.

Offered at New Rochelle Campus: 
- Spinal Cord Stimulator implants and Basivertebral nerve ablation

Expected salary and benefits

    PGY-5 salary for the 2025-2026 academic year is $99,078.

Application Information

We’re seeking fellows who will shape the future of healthcare.

Our Pain Fellowship program participates in the National Resident Matching Program (NRMP) for fellowships. Applications are accepted only through the Electronic Residency Application System (ERAS). Applications are typically accepted on ERAS from December to April of the application cycle.
